Job Description

Summary

The Patient Aligned Care Team (PACT) is a comprehensive team which delivers primary care to veteran patients in a longitudinal rather than episodic fashion and which has as its focus: prevention; health promotion - coordination - and chronic disease management. The social worker functions as a PACT teamlette member with a panel of veterans to ensure that health care meets their needs - as defined by the veteran.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
  • Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)
  • Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited
  • A doctoral degree in social work may not be substituted for the master's degree in social work
  • Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level
  • Exception: VHA may waive the licensure or certification requirement for persons who are otherwise qualified - pending completion of state prerequisites for licensure/certification examinations
  • This exception only applies at the GS-9 grade level
  • For the GS-11 grade level and above - the candidate must be licensed or certified
  • English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005 - Part II - chapter 3 - section A - paragraph 3j - this part

  • In addition to the experience above - the candidate must demonstrate all of the following KSAs: Ability to work with Veterans and family members from various socioeconomic - cultural - ethnic - educational - and other diversified backgrounds utilizing counseling skills
  • Ability to assess the psychosocial functioning and needs of Veterans and their family members - and to formulate and implement a treatment plan - identifying the Veterans problems - strengths - weaknesses - coping skills - and assistance needed
  • Ability to implement treatment modalities in working with individuals - families - and groups to achieve treatment goals
  • This requires judgment and skill in utilizing supportive - problem solving - or crisis intervention techniques
  • Ability to establish and maintain effective working relationships and communicate with clients - staff - and representatives of community agencies
  • Fundamental knowledge of medical and mental health diagnoses - disabilities - and treatment procedures

  • GS-11 Social Worker - Full PerformanceExperience and Licensure: Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings - (VA or non-VA experience) and licensure or certification in a state at the independent practice level
  • OREducation: In addition to meeting basic requirements - a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.Demonstrated Knowledge - Skills - and Abilities: In addition to the experience above - candidates must demonstrate all of the following KSAs: Knowledge of community resources - how to make appropriate referrals to community and other governmental agencies for services - and ability to coordinate services

  • The social worker is assigned to Primary Care and covers Urgent Care and Specialty Clinics as well - where appropriate
  • The Homeless Patient Aligned Care Team (H-PACT) is available without appointment or assigned PCP to veterans who identify as homeless or marginally housed with the goal of engaging them health care in the primary care clinic
  • The social worker assists the multidisciplinary team in providing care to chronically homeless veterans who are on the streets and in shelters and other temporary housing situations who are eligible for VA care but who have not established ongoing primary care in the VA system.Duties for the position include but are not limited to: Works independently with veterans and their families who are experiencing a wide range of complicated medical - psychiatric - emotional - behavioral - and psychosocial problems
  • Establishes and maintains effective therapeutic relationships with - and assesses and treats the complicated psychosocial problems of - veterans and their families
  • Provides psychosocial care of medically complex patients and offers consultation to colleagues and students on the psychosocial treatment and care coordination of complex patients
  • Participates as a member of the interdisciplinary treatment team to formulate and implement treatment plans identifying the patients' problems - strengths - weaknesses - coping skills and any assistance needed
  • Receives referrals from any member of the interdisciplinary team - from veterans and/or their families who call or walk-in for assistance - and from community professionals
  • Conducts thorough assessments to determine psychosocial problems that cause distress - often impacting the health condition and creating barriers to care needs of veterans and family members - the underlying causes of the presenting problem - the interpersonal and environmental factors impacting the problem - and its effect on the patient's ability and desire to comply with the treatment recommendations by the multidisciplinary team
  • Provides interventions to increase access to care - including transportation assistance and advanced directives - economic assistance - finding the appropriate housing for the patient's level of need (affordable independent housing - homeless shelters - group and family care homes - CalVet Homes - assisted living - nursing homes - respite care - hospice care) - and order necessary services to assist with functional decline
  • Provides ongoing supportive counseling to assist veterans in dealing with the psychosocial impact of coping with homelessness and understanding barriers to housing - with an ultimate goal of being housed and gainfully employed - when possible
  • Provides wellness/prevention education and stress management - which includes knowledge in Whole Health - facilitates patient/family support groups - and participates in shared medical appointments as appropriate
  • Provides case management interventions for veterans with members of the interdisciplinary team in order to coordinate a collaborative effort to meet the agreed upon goals for veterans' treatment needs
  • Facilitates community placements through collaboration with veterans and their families as well as interdisciplinary treatment team members to ensure that appropriate community placements are completed in a timely manner
  • Serves as a liaison between veterans - their families - the VA - and community resources in order to ensure thorough delivery of services
