Per Diem LVN Home Health / Licensed Vocational Nurse

Overview:

The Licensed Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. You will follow the plan of care, provide treatments, and collaborate with the team to achieve positive patient care outcomes.

Duties & Responsibilities:

• Provide direct patient care as defined in the State's Nurse Practice Act
• Implement the plan of care initiated by the registered nurse
• Accurately and timely document patient care consistent with the plan of care
• Educate and provide information to patients, families, and caregivers relevant to the diagnosis and plan of care
• Coordinate home health services and report needs for other disciplines
• Effectively and efficiently use equipment and supplies
• Participate in personal and professional growth and development
• Perform other duties as assigned by the registered nurse

Job Requirements:

• Graduate of an accredited practical nurse or vocational nursing program
• Three years of nursing experience preferred, with a preference for community/home health or medical/surgical experience
• Current license as a licensed practical nurse or licensed vocational nurse in the state
• Compliance with accepted professional standards and practice
• Good verbal and written communication, and organizational skills
• Must be a licensed driver with an insured automobile in good working order
• Must possess and maintain current CPR certification
